On this page you will find various forms used in the UUF. In many cases, you have two choices: Microsoft Word or Adobe PDF format. For instructions, please see below.

Microsoft Word version: If you have Microsoft Word, you can download the Word version of the form and type your information into the spaces provided.
- Right click on the ‘click’ link for the Word document that you want.
- In the menu that pops up on your browser, click the ‘Save target as…’ item
- Another window will pop up where you then specify the directory into which the file should be saved.
- Once the file has been downloaded to the directory that you specify, open the file with Microsoft Word editor
- Type in your details in the spaces provided, and when done, save your document
- Either print out the finished results, and/or e-mail your completed form to the appropriate party.
Adobe Acrobat version: View the Adobe Acrobat version of the file, print it, and then fill out the printed form. You will need Acrobat Reader on your computer in order to view them. Download Adobe Acrobat Reader.
- Click the ‘click’ link for the Adobe Acrobat version of the desired file.
- The file will be displayed in your browser.
- Use your browser’s ‘Print’ function to print out a copy of it.
